Call the City of North Salt Lake Public Works at (801) 335-8726. Your name will be put on a list, and a member of the streets crew will come to your house for an inspection. Depending on the severity of the situation, the concrete may be replaced, lifted, grinded, or nothing may be done. All work will be done by City crews or a City contractor.
